Consents can be indicated as "Cryostorage Consents" on the Digital Signatures page in your clinic settings. Marking a consent as so will allow you to indicate the validity period of storage as related to the consent, and connect them to your stored samples.
How do I set up cryostorage expiration in relation to consents?
Find the relevant consent on the Digital Signatures page
Select "Cryostorage consent"
Indicate the storage validity options related to the consent by clicking "+ Add cryo validity"
Sample type – indicate if this is valid for oocytes, sperm, or embryos … for multiple sample types, add another cryo validity field
Years reference date – choose whether the validity period begins from the sample freeze date, or the signing of this consent
Field UUID – this dropdown indicates from which field the validity period will be pulled within the document. For example, if your consent contains a field titled "Indicate the number of years to which you consent to storage," this is the field you would likely select as the Field UUID.
Click "Save"

Find the relevant consent on the Digital Signatures page
Under 'Document type,' click "Dispensation form" or "Statutory notice"
Click "Save"
How do I use cryostorage consents?
When sending consents
When a consent has listed sample validity dates as configured above, the platform will display the validity periods indicated in the consent for your confirmation.
Once a patient completes the consent form, you will be able to review – and edit, need be – then confirm the cryostorage expiry dates.
When linking them to samples
Visit the sample to which you would like to link a sample, and scroll down to the 'Consents' header. Click "Actions" > "Link consent" and select which consent form(s) you would like to link to the sample, then click "Save."
