Consent form status

What this card shows

The Consent form status card counts consent forms by status, month by month, using the month each form was issued. There are two lines:

  • Pending: issued, awaiting signature.

  • Signed: signed by the patient.

Use it to track your consent backlog and to see how quickly issued forms come back signed.

Reading the card

  • Each month on the horizontal axis is the month the form was issued, not the month it was signed. A form issued in April and signed in June counts in April, as signed.

  • Because of that, the Pending line in the most recent month or two is not really a backlog. Those forms have only just gone out. The backlog to worry about is a Pending count that stays high in months that are well past.

  • Hover any point to see the exact number of forms.

  • The card counts forms, not patients. A patient with several forms contributes several.

The card lives on the Nursing & care team dashboard, on the Consents & surveys tab, and responds to the filters at the top:

  • Date: change the time window. It scopes by issue date, and defaults to the past 6 months plus the current month.

  • Clinic: focus on one or more locations, if your organization has several. It keeps forms whose patient has a cycle at the selected clinic, so a patient treated at more than one clinic is counted under each.

Where this data comes from

Consent forms live on the Consents tab of the patient's record. A form appears here as pending as soon as it is issued to the patient, and moves to signed once their signature is recorded.

Issuing forms through the patient's record, rather than handling them outside wawa, is what puts them on this card in the first place.

Opening this card

In wawa, open Insights from the sidebar, then go to Standard Reports → Nursing & Care Team → Nursing & care team. Select the Consents & surveys tab. The Consent form status card is on it.
