Questionnaire patients to contact

What this card shows

The Questionnaire patients to contact card is a working list of patients who were sent a questionnaire and have not completed it yet. Each row is one patient with at least one questionnaire still outstanding.

It is a table you can work down rather than a chart to interpret.

Reading the card

The table has one row per patient, with these columns:

  • Patient: the patient's name.

  • Clinic: the clinic the questionnaire belongs to.

  • Questionnaire: which forms are still outstanding for them, by name. A patient with more than one incomplete questionnaire shows all of them.

  • Last requested: the date of the most recent questionnaire sent to them.

  • Days waiting: counted from their oldest incomplete questionnaire, not the most recent one. A patient sent two questionnaires shows the newest date under Last requested and the older one's age under Days waiting.

Rows are sorted longest-waiting first, so the top of the list is where to start.

The card lives on the Nursing & care team dashboard, on the Satisfaction tab, and responds to the filters at the top:

  • Date: the window the questionnaires were sent in. It defaults to the past 6 months plus the current month, so patients who were sent a questionnaire before that window are not listed. Widen the filter if you want to see everyone still outstanding.

  • Clinic: focus on one or more locations, if your organization has several. It keeps patients who have a cycle at the selected clinic, so a patient treated at more than one clinic is counted under each.

A patient drops off this list as soon as they submit the questionnaire.

Where this data comes from

Questionnaires are set up under Settings → Surveys, where each one can be given a trigger: sent automatically before or after an appointment of a given type, sent at onboarding, or left to be sent by hand. Whatever the trigger, everything sent to a patient appears on their Surveys tab, and staff can send one from there with Send Survey.

The tab lists each questionnaire with who it went to, the date it was sent, and the date of completion. The patient fills it in through the patient portal, and until they submit it the questionnaire stays on this card. The Surveys tab is also where you check what a patient has already answered before chasing them.

Opening this card

In wawa, open Insights from the sidebar, then go to Standard Reports → Nursing & Care Team → Nursing & care team. Select the Satisfaction tab. The Questionnaire patients to contact card is on it.
