Surveys allow your patients to share information about their health, past treatments, demographics and more prior to appointments.

1. Surveys in wawa

Sending surveys through wawa allow you to gather important information about your patients prior to appointments – and allows your patients to choose when and where to complete these surveys at their convenience.

2. How Do Surveys Work?

Getting Surveys into the Platform: To create a survey, visit your Clinic Settings Page, and navigate to the “Surveys” tab on the left-hand side. Click “New Survey” to open the survey builder.

Note: The survey builder may take a moment to open, this is normal.

The “How” of Surveys in the Platform

Create a New Survey from Scratch:

  1. Open the survey builder by clicking “New Survey”

  2. Click “add a question” to get started

  3. Give your survey a title and description

  4. Play around with the question types to best collect your information

  5. Add logic to your survey questions under the “Conditions” field in the question settings, or the “Logic” tab

  6. Save your survey before exiting the survey builder

Create a New Survey from the Base of a Different Survey:

  1. Download the survey you wish to use as a foundation using the “JSON Editor” tab

  2. Click “New Survey”

  3. Open the “JSON Editor” Tab

  4. Upload the downloaded survey via the folder icon

  5. Edit this survey in line with your needs

Editing a Survey: Edit any survey through the survey builder in the same manner it was created!

What Patients See in the App

  1. Surveys are shown on the home page or in the documents tab of the app

  2. Patients complete the survey in the app

  3. Patients hit “Complete” once done

Note: Consents must be completed in one sitting. Progress cannot be saved, nor restored.

Once a Patient Submits a Survey: View their answers through the ‘Surveys’ tab on the left-hand side of their patient profile.
